Accessibility Modification Agreement

Resources for Rural Community Development (AccessHome), acting on behalf of Far Northern Regional Center (FNRC) consumers, fund Accessibility Modification project(s). The Executive Director of AccessHome or any other individual designated by the AccessHome Board of
Directors will approve the project(s). AccessHome may pursue other funding sources to help
pay for the project(s). AccessHome will provide a letter of commitment to the applicant
outlining the specific improvement items that will be funded. In consideration of the above,
the applicant, the property owner and/or the consumer representative agrees to the
following terms and conditions:

1. At all times the applicant agrees to cooperate with AccessHome in their project review efforts, as well as any other funding source obtained by AccessHome, for the proposed Accessibility Modification.
2. The applicant will provide three (3) modification construction bids to AccessHome to
assist in review and evaluation of the proposed project(s) prior to the issuance of any
letter of commitment concerning funding.
3. The applicant agrees to cooperate with any additional modification construction bids,
which AccessHome or other available funding sources, deem necessary.
4. The applicant will execute any and all agreements necessary for the modification
project(s) and will be primarily responsible to any retained and approved licensed and bonded contractor.
5. AccessHome will make all payments directly to the contractor upon completion of project
and approval by any required governmental authority and the consumer.
6. Any amount(s) claimed by the approved contractor in excess of the bid approved by AccessHome, either due to change in material costs or change orders authorized by the applicant, will be the responsibility of the applicant and not AccessHome unless also approved
by AccessHome in advance.
