Accessibility Modification Funding Guidlines

AccessHome is a non-profit corporation that provides accessibility services to developmentally disabled consumers in northern California. AccessHome is the payer of last resort; all other
funding sources need to be researched prior to referral.

Prior to AccessHome referral, the Service Coordinator must discuss the situation with the
Case Management Supervisor. AccessHome will not accept referrals without the Case Management Supervisor’s signature on the referral letter. The list below is meant to be a
list for Service Coordinators to use when determining whether AccessHome programs can
help a consumer. Services listed as provided by AccessHome may not be provided to each consumer, as services are provided based on the consumer’s needs, not desires. Each referral AccessHome receives will be handled on an individual basis to determine if a service will be provided. AccessHome will limit assistance for accessibility modifications for consumers to one approved project in a five-year period. AccessHome will provide only minor modifications to
rental property.

Provide permanent ramps
Cover some construction/installation costs for specialized equipment (i.e. lift stations, reconfigure kitchen and/or accessibility of appliances for wheelchair usage)

Provide portable ramps
Provide specialized equipment
Add rooms or square footage to a residence
Build fencing
Perform general household repairs
Install heating and cooling system upgrades
Pour sidewalks
Purchase or install new carpet which is more wheelchair friendly
Provide any home upgrades to meet new building codes
