Accessibility Modification Policy

It is the intent and policy of AccessHome to help provide accessible housing to
developmentally disabled consumers of the Far Northern Regional Center. This will include modifications for consumers and/or families of developmentally disabled children. AccessHome
will provide adaptations to the consumer’s home as needed to implement an objective in the individual’s IPP. All services shall be provided in accordance with applicable State or local
building codes.

Scope of Work - shall be deemed necessary to ensure the health, welfare and safety of the consumer and/or enable the consumer to function with greater independence in their home.
Such adaptations may include:
Home or bathroom entrance: door widening for wheelchair access
Wheelchair ramps, grab bars, handrails to aid ambulatory problems
Modification of bathroom facilities-wheelchair accessible shower unit and installation
Installation of specialized systems necessary for the welfare of the consumer

Structural Modifications - excluded are adaptations or improvements to the home which
are of general utility, and are not of direct medical or remedial benefit to the individual (i.e. carpeting, roof repair, central air conditioning, etc.). Adaptations/modifications that add to
the total square footage of the home are excluded.

It is not the policy to complete any remodeling and/or additions to existing structures, unless deemed necessary to complete an accessibility modification and within policy financial limit.

Any modification to be completed on rental property must have the owner’s written
consent.

Time Line
AccessHome will limit assistance for accessibility modifications for consumers to one approved project in a five-year period.

Financial Commitment per Project
The decision of which projects to approve and the amount of monies to be allocated to each
individual project will be determined by potential long term usage, consumer’s need, and availability of funds. The amount spent on any project will be at the discretion of the
Executive Director and Board of Directors of AccessHome. The Board of Directors has
established a limit on financial commitment to each project. All monies allocated are on a
first-come, first-served basis.

Any request to adjust and/or appeal this policy must be submitted in writing to, and approved by AccessHome Board of Directors at a regularly scheduled board meeting.
